The present Neuro conference focuses on Neurological disorders that are diseases of the brain, spine and the nerves that connect them. There is ample evidence that pinpoints neurological disorders as one of the greatest threats to public health. There are more than 600 diseases of the nervous system, such as Alzheimer's diseaseBrain tumoursEpilepsyParkinson's disease, and Stroke as well as less familiar ones such as front temporal dementia. The molecular basis of Neuropharmacology with a focus on clinical applications and mode of action of drugs is explained as drug receptor interaction. In Neuropediatrics department the variety of patients seen by a Child Neurologist varies from those with common, relatively straightforward conditions, such as cerebral palsy or migraine, to those with rare or complex conditions, such as metabolic or degenerative disorders. Neuroimmunology, the study of the interaction between our central nervous system (the brain and spinal cord) and our immune system, melds these two disciplines.

Europeans are plagued by mental and neurological illnesses, with almost 165 million people or 38 percent of the population suffering each year from a brain disorder. With only about a third of cases receiving the therapy or medication needed, mental illnesses cause a huge economic and social burden, it found the cost burden of these and neurological disorders amounted to about 386 billion euros ($555 billion) a year. An estimated 6.8 million people die every year as a result of neurological disorders. Neurological disorders public health challenges describes and discusses the increasing global public health importance of common neurological disorders such as dementia, epilepsy, headache disorders, multiple sclerosis, neuroinfections, neurological disorders associated with malnutrition, pain associated with neurological disorders, stroke and traumatic brain injuries.

Track 1: Pediatric Neuropathy

Pediatric neuropathy is most common disorder in children. The symptoms of this disorder include weakness, sensory loss (numbness) and positive sensory symptoms such as paresthesia, pain and burning sensations. The diagnosis of a peripheral neuropathy can be done based on the history taken and the clinical examination.  Peripheral Neuropathy is a disorder of peripheral nerves and 70% of chronic neuropathy in children is hereditary, 20% is indeterminate and 10% is acquired.  This type of Neuropathic pain is studied rarely in infants, children, and adolescents. The Diabetic peripheral neuropathies (DPN) are major difficulties of insulin-dependent diabetes mellitus (IDDM). For every year there are about $ million babies born with a genetic disease or major birth defect. For example In USA around 15,000 babies are diagnosed to have Huntington’s disease

Track 2: Autonomic Neuropathy

The autonomic neuropathy (AN) is defined as damage of nerves which can help for organs and organ systems. This damage of nerve leads to disturbance in signal processing between the autonomic nervous system and the brain.  Autonomic neuropathies are combination of syndromes and diseases which affect the autonomic neurons like parasympathetic and sympathetic and they can be hereditary or acquired in nature. The degree of autonomic dysfunction may be subclinical or clinically irrelevant in some patients.  The  common causes of autonomic neuropathy includes Abnormal protein buildup, Autoimmune diseases, Diabetes, Injury to nerves, Treatment with certain medications, Certain infectious diseases and Inherited disorders. The risk factors for Autonomic neuropathy are diabetes, alcoholism, lupus, HIV, Parkinson’s disease, botulism, cancer.

Track 3: Neurology

Neurology is concerned with the study and treatment of nervous system disorders.  Neurologists are physicians who are specialized in neurology and they can treat disorders of the brain, spinal cord, and nerves including pain and muscular disorder, headache, epilepsy, neuritis, brain and spinal cord tumors, multiple sclerosis, stroke, Parkinson's disease, myasthenia gravis, muscular dystrophy. To diagnose or to treat a condition neurologists participate in a variety of procedures includes Lumbar Puncture, Tensilon Test, and Electroencephalogram. They may use magnetic resonance imaging (MRI), computed tomography (CT) scans, positron emission tomography (PET scan) for diagnosis. To care for all patients with neurologic diseases, the department of Neurology works closely with the department of Neurosurgery. According to estimates by the WHO the neurologic disorders affect over 1 billion people worldwide. Among this 12% of the global burden of disease and it cause 14% of global deaths. Moreover in the past 10 years, the number of deaths caused by neurological diseases has more than doubled and this will be increased in the upcoming years.

Track 4: Neuropharmacology

Neuropharmacology deals with study of how the drugs will affect cellular function of nervous system and neural mechanisms. It will also discuss drug-induced changes in functioning of the nervous system. It is a broad portion of science and that deals many aspects of nervous system from single neuron manipulation to entire areas of the spinal cord, brain and peripheral nerves. Neuropharmacology is very active field of research, because effective treatments available to treat psychiatric or neurological disorders with major public health implications.

Neuropediatrics is deals with neurological disorders like disorders of the brain, spinal cord, peripheral nerve and muscle affecting infants, children and adolescents. It is also called as Pediatric Neurology or Child Neurology. The child neurologists divide those disorders in to common, relatively straightforward conditions, such as cerebral palsy or migraine and complex conditions, such as metabolic or degenerative disorders. Child neurologists often diagnose, treat, and manage the conditions like Seizure disorders, including seizures in newborns, febrile convulsions.

Neuroimmunology is a field of neuroscience, which combines the study of the nervous system and immunology and immune system. Neuroinflammation and neuroimmune activation have major role in the study of neurological disorders such as stroke, multiple sclerosis, pain, and AIDS-associated dementia and Parkinson's and Alzheimer's disease. Neuroimmunologists will diagnose and treat neurological disorders caused by inflammation or autoimmunity in the nervous system.  The main goal of this field is produce specialists with the knowledge and tools to diagnose treat and prevent immune-mediated disorders of the nervous system.

Track 7: Neurophysiology

The study of neurons, nerves and nervous systems such as what they do and how they do it is called as Neurophysiology. A nerve is defined as bundle of axons originating in various cells and aggregation of these nerve cells within a single organism is referred as nervous system. In case of evolution, all the simplest animals contain the use of nervous system to organize and carry out their behaviors. So, this type of behavior can be studied in neurophysiology which is a part of neurology. The role of receptors is to receive signals from the external environment or from the internal environment, i.e., signals about the condition of the body itself. The receptors sensitivity can be variable between genders and individuals. In Europe the increasing prevalence of neurological disorders is the main factor driving the neurophysiological monitoring (NPM) devices market. The market earned revenues of $132.2 million in 2006 and estimates this to reach $275.5 million in 2013.

Track 8: Central Nervous System

The nervous system of human body is divided into central nervous system (CNS) and the peripheral nervous system (PNS). The CNS contains Brain and spinal cord. The brain plays vital role in the control of body functions, speech, memory, sensations and movements and spinal cord connected to the brain through brain stem which carries signals back and forth between the brain and the peripheral nerves. The Cerebrospinal fluid circulates within the cavities of central nervous system. The Cerebral Hemispheres are the largest and most obvious parts of the human brain. The neuron or nerve cell is the basic unit of CNS and they allow different parts of the body to communicate with each other via the brain and the spinal cord.

Track 9: Minimally Invasive Neurosurgery

Minimally invasive surgery is used by neurosurgeons and mostly they use non-invasive treatments whenever these techniques can achieve better or comparable results compared to standard open surgical procedures. Minimally Invasive Procedures includes Microcraniotomy and Neuroendoscopy. The main goal of this surgery is to reduce tissue disruption and thus morbidity. This is a review of the development, applications, benefits of minimally invasive neurosurgery and its wider surgical implications. To visualize various parts of the brain, skull base, or spinal cord through small openings, Minimally invasive neurosurgery utilizes endoscopes which are small, flexible, lighted tubes. In case of intracranial surgery, endoscopes are introduced into the brain through very small skull openings.  Minimally invasive spine surgery uses advanced technology and techniques to treat spine condition without disrupting the surrounding soft tissues. Highly specialized tools and Computer-assisted technology are used to assist the surgeons with visualization and control.

Track 10: Pediatric Neurosurgery

Pediatric surgery is a type of surgery involving the surgery of fetuses, infants, children, adolescents and young adults. In the pediatric age group Neurosurgical problems are often difficult and complex. So, the children with nervous system problems frequently require ongoing, close follow-up throughout childhood. The Pediatric neurosurgeons treat children from the newborn period and also in the teenage years and they are often quite different from those by adult or general neurosurgeons. Pediatric neurosurgeons diagnose, treat, and manage children’s nervous system problems and head and spinal deformities including the Head deformities, Spine deformities, Problems and injuries of the brain, spine, Gait abnormalities (spasticity), Birth injuries like weakness of arms and legs.

Track 11: Spinal Neurosurgery

Spine surgery is typically an important, undertaking that it is considered as a possible approach to enhance a patient’s ability to decrease pain. Spine surgery can basically accomplish three tasks like Decompress a nerve root or the spinal cord, stabilize a painful or unstable segment with spinal fusion surgery and reduce a deformity.  In case of non-surgical treatment such as medications and physical therapy Spine surgery may be recommended which results in fails to relieve symptoms. Surgery is only recommended in cases, where the exact source of pain can be determined such as a scoliosis, herniated disc, or spinal stenosis. This can be contributed to spinal field by performing artificial disc surgery, spondylosyndesis and endoscopic spinal disc surgery.  The treatment of spine disorders at the Neurology and Neurosurgical Services utilizes the most modern concepts of both the treatment and prevention. Patients are evaluated to distinguish those who will benefit most from conservative therapy from those who will need surgery. The best test currently available is Magnetic Resonance Imaging, which is performed early and in most instances results in a complete and accurate diagnosis. CT/myelogram, commonly used in the past and is now rarely used due to the superiority of the MRI. The sophisticated MRI computer software available at Saint Barnabas for the diagnosis of spine disease alleviates the pain and discomfort previously experienced with myelogram.

Track 12: Clinical Neurophysiology

Clinical Neurophysiology is a branch of medicine with diagnostic specialty used to assess function in the nervous system by using electroencephalography (EEG), electromyography (EMG), evoked potentials (EP) and nerve conduction studies (NCS). It has medical specialty that evaluate the central and peripheral nervous systems through the recording of bioelectrical activity. This field also concerned with diagnosis and monitoring of disease by measurement of the electrical activity of the spinal cord, brain, muscles and nerves. By some rheumatologists and Electrophysiology It is also called Electrodiagnostics (EDX). In some countries like USA and Germany Clinical neurophysiology is a part of neurology or psychiatry. In other countries such as Spain, Portugal, Italy, Finland, Sweden, the United Kingdom, and Norway it is an autonomous specialty. Clinical neurophysiologists are responsible for writing and analyzing the reports on tests that take place in the department and they receive and transfer this information to the doctor that referred the patient to the particular neurophysiologist.

Trauma surgery has specialty in utilizing both operative and non-operative management to treat traumatic injuries, especially in an acute setting and normally focuses on the abdominal area. In case of general surgery Trauma surgeons complete residency training. At some point every neurosurgeon is called on to treat a victim of brain or spinal cord injury in his or her career. There are about 100 to 350 patients per 100 000 inhabitants per year are admitted to hospital care following a traumatic brain injury (TBI) all over the world and among these about 80% patients are categorized as head injury. In the United States approximately 1.7 million people contain a traumatic brain injury per year and is a contributing factor to a third of all injury-related deaths.

Track 16:  Pituitary Neurosurgery

Pituitary neurosurgery is used in evaluation and treatment of pituitary tumors and other neuroendocrine disorders like prolactinomas, acromegaly, Cushing's syndrome, craniopharygiomas, nonfunctioning pituitary tumors and disorders of the hypothalamic region and pituitary that lead to growth hormone deficiency. This treatment is depends on type of tumor, location and size. Pituitary adenomas may grow to considerable size and compress important vascular and neurologic structures at the skull base. When this occurs surgery is indicated to decompress these structures especially when vision is at risk. There is another type of surgery to remove this type of tumor is craniotomy which involves removing a piece of bone from the skull by making an incision in the patient’s scalp.  Intraoperative MRI (IMRI) is one of the major challenges of using either the trans cranial approach or the transsphenoidal to pituitary tumors is the difficulty in precisely determining how much of pitutary tumor has been removed at the time of surgery.

Track 17: Operative Techniques in Neurosurgery

Advanced Neurosurgical Techniques includes Craniotomy in which small opening is made in the skull to gain access to a tumor. In this procedure in order to remove as much of the tumor neurosurgeons are guided to the precise location of the tumor and tissues from this procedure was analyzed by neuropathologist. Awake Craniotomy with Functional Brain Mapping technique is similar to the Craniotomy but patient is fully awake during the middle of the procedure.  Skull-based Surgery is important operative approach for tumors along the skull. In this patients are cared for by a multidisciplinary team that includes otolarygologists, radiation oncologists and neurosurgeons. Stealth neuro-navigational/image-guided craniotomy is a unique imaging with computer guidance using the microscope for precise tumor removal. In Stereotactic brain biopsy technique,  a CT or MRI scan and a computer system is used  to assist our neurosurgeons in pinpointing the precise location of the brain mass so that a small amount of tumor tissue can be removed which can be used for diagnosis.

With only about a third of cases receiving the therapy or medication needed, mental illnesses cause a huge economic and social burden, it found the cost burden of these and neurological disorders amounted to about 386 billion euros ($555 billion) a year.An estimated 6.8 million people die every year as a result of neurological disorders. As per Neuropharmacology, the pathomechanism of neurologic disorders becomes better understood, the focus is on development of neuroprotective rather than symptomatic treatments, particularly for neurodegenerative disorders, stroke, and CNS trauma.

Neurological disorders public health challenges describes and discusses the increasing global public health importance of common neurological disorders such as dementia, epilepsy, headache disorders, multiple sclerosis, neuroinfections, neurological disorders associated with malnutrition, pain associated with neurological disorders, stroke and traumatic brain injuries.
