Diabetes often referred as diabetes mellitus, describes a group of metabolic diseases. Diabetes occurs when there is a dis-balance between the demand and production of the hormone insulin. There are different types of diabetes and all types of diabetes are treatable. Pathophysiology of  diabetes leads to autoimmune disease and its also known as IDDM,  whereas it also leads to the Peripheral insulin resistance and also known as NIDDM or “adult onset” diabetes. Pathophysiology of gestational diabetes is also one of the types when there are excessive counter-insulin hormones of pregnancy. Every few years, the  metabolic syndromes like diabetes is  re-evaluates by community  the current recommendations for the classification, diagnosis, and screening of diabetes, reflecting new information from research and clinical practice which in turns help in understanding current prevention and treatment options and cost effectiveness in treatment and prevention of Diabetes.

It is important to diagnose the diabetes and to evaluate it clinically as soon as possible. The expertise recommends that everyone aged 45 and over should be clinically analysed for diabetes, and if the results are normal. But due to change in food habbit, Childhood diabetes is also becomming common amongs childrens. Traditionally, diagnosis of diabetes should be made by using a blood glucose meter and finger prick, or by urine glucose test in clinical laboratory. Testing enables health care providers to find and treat diabetes before complications occur and to find and treat prediabetes, which can delay or prevent type 2 diabetes from developing. To treat the disease of diabetes, insulin is produced using recombinant DNA technology.  The an A1C test, also called the haemoglobin A1c, HbA1c, or glycohemoglobin test ,a fasting plasma glucose (FPG) test, an oral glucose tolerance test (OGTT) can be used for diagnosis. The gene therapy is also one of the best methods to be considered for treating the diabetes.  Clinical trial on animal and various  latest research on diabetes are being practised.

Case study is an important part for research and treatment. Clinical case study aims directly improve global health outcomes and share clinical knowledge using case reports to convey important best practice messages. If case study is done then the advanced medical technologies like transplantation, holistic treatment can be used more rapidly in hospitals and with the more study in Glycemic index and load  we can find the diseases or problem like Cortisol . Even with this, we can know the number of people by using Diabetes Statistics having  diseases and can find by advances in diabetes cure. Drug mechanism can be known and along with these new technologies as well pharmacotherapy can be done. Clinical case study done in   medical diabetes  could leads to find innovative way for treating the diabetes.

Diabetes is a metabolic disorder  in which the person has high blood glucose (blood sugar), either because insulin production is inadequate. Long-term complications of diabetes like diabetic cholestrol develop gradually. The longer we have diabetes, and the less controlled our blood sugar, the higher the risk of complications to skin & immune sytem , cardiovascular diseases, and nerve damage. Diabetes and alcohol will cause your blood sugar to rise and alcohol has a lot of calories. Patients with diabetes will typically experience various complications like Diabetic Macular Oedema , people with diabetes also facing one of the major complication that is diabetic foot having problems like ulceration, polyuria, infection or neuropathic osteroarthropathy, polydipsia, Alzheimer's disease, polyphagia and even more. Diabetes being the one of the causes of death in world and results into other health complication caused like blindness and kidney failure.

The main goal of diabetes management is to restore metabolism of carbohydrate to as close to a normal state as possible. To achieve this goal, individuals with an absolute deficiency of insulin require insulin replacement therapy, which is given through injections or an insulin pump. Insulin resistance, in contrast, can be corrected by dietary modifications and exercise. Diabetes is a metabolic disease which can be corrected by transplantation techniques. This different type of diabetic disease can be prevented by islet transplantation or even by artificial pancreas transplantation and some time Hypoglycemia Awareness  and checking sugar level regulary is necessary. Prevention during pregnancy is very crucial and meanwhile we should know the way for managing the gestational diabetes. Diabetes management are to prevent or treat the many complications that can result from the disease itself and from its treatment. The metabolism boosters  gets changes. Modern approaches to diabetes primarily rely upon dietary and lifestyle management, often combined with regular on-going blood glucose level monitoring. Diet management allows control and awareness of the types of nutrions entering the digestive system, and hence allows indirectly, significant control over changes in blood glucose levels

The concept of and new discovery and emerging therapies like reverse diabetes  for the treatment of diabetes has expanded in recent years. There is a lot of latest research on diabetes happening all around the world and the latest includes  Some very rare forms of diabetes are caused by single gene mutations. They are called "monogenic" forms of diabetes and new research is opening the door to specific treatments for these patients. With the advanced  type1 diabetes treatment  various devices e.g. insulin pumps which is used for the administration of insulin , rapid HbA1c monitoring, insulin pen, biomarkers, biostator, artificial pancreas and etc. are formed for the treatment. Various pharmaceutical companies are making drugs for improving the lives of those with the disease. Number of innovation and treatment are increasing with the increase in research work by Research and development department in various sectors and endocrinology is getting treatement with these devices. Continuous Glucose Monitoring (CGM) is a relatively new technology which has the potential to assist people living with type 1 or type 2 diabetes and treated with insulin to achieve the goal of optimum control of blood glucose.this will remove the diabetes cholestrol.

The research on diabetes leads to focus more on the diagnosis and treatment of diseases with the new approaches yet to be discovered or under developments. Maturity onset diabetes of the young, Diabetic Dyslipidemia is one of the major issues for the concern which is due to mutation. Without the pancreas producing this insulin, the sugar will stay in our blood and begin to create serious health issues. High blood sugar can cause blindness, kidney failure, nerve damage, and other organ function problems. There are various metabolic  syndrome  which are to be treated with recent tools for obesity and weight management  is  Bariatric surgery , stem cell therapy. With the development of software’s, now the computational tools i.e bioinformatics are used for the analysis of data e.g. Sequence alignment, microarray analysis and specific tools mentioned. The researcher are also focusing on the metabolism of people with diabetes as it differs to the metabolism of people without diabetes.  In type 2 diabetes, the effectiveness of insulin is reduced and in type 1 diabetes, insulin levels in the body are very low. There is a strong correlation between BMI and body fat. One of the correlates of insulin resistance is the blood fatty acid (FA) level. FA levels are strong predictors of muscle insulin resistance. In diabetes for dummy's, it is vital to get tested regularly to make sure your normal blood sugar levels are safe. In the meantime, diabetes for dummies says to check your blood sugar levels, and know what they are.  Electron microscopy shows decreased mitochondrial size in muscle from individuals with type 2 diabetes.

Diabetes fall into the categories of type 1, type 2, and diabetes that occurs during pregnancy (gestational diabetes). However, up to 5% of cases have other specific causes and include diabetes that results from the mutation of a single gene. Maturity onset diabetes in the young (MODY) that is inherited as an autosomal dominant trait is an uncommon cause of diabetes that may be mistaken for type 2 diabetes. The clustering of cardiovascular risk factors such as obesity and diabetes , hypertension, dyslipidaemia and glucose intolerance in the same persons has been called the metabolic or insulin-resistance syndrome.The more typical, common and multi-factorial form of Type 2 diabetes results from the interaction between environmental risk factors and predisposing genotype (a combination of genetic variants). (T2D) is characterized by insulin resistance in peripheral tissues like muscle, fat and liver coupled with effects of aging, obesity and reduced exercise  and dysregulated insulin secretion by pancreatic betacells. In T2D, the pancreatic disorder like  pancreatic beta cells become progressively less able to secrete sufficient insulin to maintain normal carbohydrate and lipid homeostasis.

The National Institute of Health estimates that obesity and related complications such as diabetes are our nation's chief health concern. However, while most people tend to think of diabetes and obesity as a developed market problem, the two issues are spreading quite rapidly across the globe. Roughly, four out of five people suffering from diabetes are now located in developing markets. China alone, has 92.3 million diabetic citizens, while India’s diabetic population is over 50 million.
